Terms & Conditions

Please take some time to read this information so you are fully informed and have the opportunity to ask questions if required.

 

These Terms and Conditions form part of your consent that allow us to provide you treatment.

1.1 You give you consent to receive a physiotherapy assessment and treatment, including the necessary examination and physical contact in the relevant area(s).

1.2 You understand that procedures and treatment options will be explained to you and you will be given the opportunity to ask questions.

1.3 You understand that you may withdraw your consent at any time. Treatment will then immediately cease.

1.4 During your treatment we may ask to photograph / video your postures / movements or record exercises you need to undertake at home. If you do not consent to this, please advise your physiotherapist. These are for your personal use only and will not be shown / shared with any other parties.

1.5 If you are willing to have a photograph taken for our website / one of our social media channels, then we will require you to sign a release form allowing us to do so. Should you change your mind then this will be removed within 72 hours of a written request having been received

1.6 You are aware that you may bring a chaperone with you to the appointment should you wish (during the COVID-19 pandemic a chaperone is only permitted if medically required / for children under 18 years of age).

1.7 In the event that a member of our Team is off sick or away, you understand that you may be seen by another member of our Team to avoid interruption to my treatment.

2. Funded or referred patients

For patients whose treatment is via a referral or funded by a third-party, you acknowledge that insurance-covered session charges may vary from our ‘standard rates’ and as such we are unable to invoice insurance companies retrospectively on your behalf. You also agree to the following:  

2.1. You understand that as part of your treatment, you consent to allowing any reports regarding your assessment and treatment to be released to your funding body.

2.1.1 This may also include clinical notes in some cases. We will obtain written consent from you before disclosing these notes, except in cases where consent is provided by the referring party either by way of contract or prior agreement with you.

2.2 To provide the following details to The LBPC prior to commencing your course of treatment:

  • Health insurance company name and contact details.

  • Health insurance policy number.

  • Insurance authorisation code and the number of authorised sessions for the current course of treatment and the number of authorised sessions.

  • The amount of any excess and whether the insurer will claim this back via The LBPC.

2.2.1 You understand that failure to provide complete and accurate insurance authorisation details prior to the commencement of your course of treatment means that you will be required to pay for treatment at The LBPC ‘standard rates’ prior to or during every session in accordance with our terms and conditions.

2.2.2 Once a course of treatment has commenced The LBPC is unable to invoice insurance companies on your behalf and our standard rates will apply to all sessions within that treatment course.

3. Late Cancellations & Missed Appointments

3.1. You agree that if you are unable to attend a booked appointment, you will provide at least 24 hours notice, wherever possible.

3.2. If you do not attend an appointment or cancel with less than 24 hours notice, you agree to the following:

3.2.1. You may be liable to a cancellation or missed appointment fee.

3.2.2. For Funded or Referred patients (as described in point 2), the funding or referring party may be advised of the cancelled or missed session and this may affect continuation of your treatment.

3.3 For appointments booked with less than 24 hours' notice, points 3.1 and 3.2 do not apply. However, notice of a cancellation in these cases should be provided as soon as possible.

Cancellations

4. Charges and Payments

4.1 You will be charged for your appointment in line with the rate current at the time of booking. This can be obtained by contacting us.

4.2 Payments can be made using any of the following methods:

4.2.1 Online Electronic Payment

4.2.2 Credit Card

4.2.3 Debit Card

4.3 Payments can be made in advance, or will become due immediately following the appointment.

4.4 Delayed payments will be subject to our debt recovery process, including any interest accrued.

4.5 Patients are requested to check the conditions of their Health Insurance Policy and/or referring party, as we cannot be held responsible for any non-payment of claims. If insurers are unwilling to make payment, you will be liable for all costs.

5. Valuables

5.1. You understand that The LBPC is not responsible for the loss or damage of any personal items (including valuables and jewellery).

5.2. We advise that any valuable items, such as jewellery, not be taken to or worn at our clinic.

6. Clinical Notes / Forms

6.1 Before your initial assessment you will be emailed forms to complete.  It is your responsibility to disclose to us all details requested.  Failure to disclose this information may adversely affect your treatment. By accepting and signing our terms and conditions you agree that all information provided in the forms are correct and consent to treatment at The LBPC.

6.2 You give your consent to your physiotherapist filling in any additional forms on your behalf that may be required during a treatment session. By accepting and signing our terms and conditions you agree that the verbal information  you give is correct and true.

6.3 You understand that you have a right to request a copy of your clinical treatment notes, which can made by contacting us.

6.4 You understand that there may be an administration charge for this service.

Clinical
Student Discount

7. Student Discount

7.1 We reserve the right to determine eligibility for the student discount in our sole discretion. To qualify for the discounted price, you will need to submit a valid Student ID prior to your first consultation as the discount cannot be added retrospectively.

7.2 A valid form of Student ID is defined as being issued by one of the following:

  • ISIC

  • NUS Extra

  • UniDAYS

  • Student Beans

  • A recognised education provider i.e School, College or University

7.3 Discount offers are personal and are non-transferable. You cannot redeem discount offers for cash. Discount offers are available for the time periods set by us in our sole discretion

7.4 By using a discount offer you: (a) acknowledge and agree to our Privacy Policy; and (b) consent to use of the information you submit to us in accordance with our Privacy Policy.

Complaints

8. Complaints & Feedback

8.1 If you are unhappy about any aspect of your experience at The LBPC, please contact The LBPC so that the matter can be resolved where possible, prevented from happening again and thus further improving our Service.
